SMI SUPPORTED HOUSING
Empowering Patients
CBI believes in utilizing “housing first” and “harm reduction” philosophies to help individuals achieve desired goals around daily living activities and access to community-based support networks. The certified peer support navigators understand the challenges associated with maintaining housing while struggling with mental health and substance use issues and they work with each individual member to achieve his or her self-defined goals.

Peer Navigators to Help along the way
Services are provided by Peer Navigators who envelop the peer values of hope, resiliency, universality and resource building. Services are person-centered and focused on assisting the member to live independently and maintain housing.
CBI’s Permanent Supportive Housing Team uses the SAMHSA evidence-based fidelity model of treatment to help individuals find affordable, safe, integrated housing options with flexible and individualized services.
Accessing Care & Services
Who is eligible?
To qualify for Mercy Care SMI Supported Housing services, members can be NT19 and/or T19 SMI Members, and identified as being in need of supplemental services by their assigned SMI provider. A member can request assistance and work with their assigned SMI provider for a referral.
