Recovery Begins at Home
CBI serves the Veteran community with wrap-around services that connect Veterans with housing and ongoing care and services. Our ultimate goal is to improve housing and income stability, along with making meaningful connections and engaging in community activities. This permanently sets the path for long-term vibrancy in the community.
CBI deploys a team of Peer Navigators to do street outreach in search of Veterans who want a new way of life. Vets may volunteer to transition to temporary and then permanent housing, via the Bridge system.

Veteran Services
Working alongside participants through the application process for Veterans Administration (VA) services is just the beginning. We continue to work with veterans to get them the services they need such as:
Employment assistance
Primary medical care such as wellness and prevention visits
VA and other application aid for housing
Medical, behavioral and recovery care and counseling
Veteran Connections Program
Peer Navigators provide outreach services to engage Veterans experiencing homelessness and work with them to connect to available services.
Supportive Services for Veteran Families (SSVF) Program
Supportive Services for Veteran Families (SSVF) is an initiative of the United States Department of Veterans Affairs (VA) to promote housing stability among very low-income Veteran families who reside in, or are transitioning to, permanent housing. The VA partners with community-based organizations and local non-profit agencies across the United States to provide rapid rehousing, rapid resolution, and homeless prevention services through evidence-based practices and case management strategies in order to achieve housing stability. To qualify for assistance with SSVF, all households must meet the following three requirements for eligibility: Verification of active duty service and qualifying discharge status (from Form DD-214, Statement of Active Duty Service, VHA Eligibility, etc.). Income resources at or below 50% of Area Median Income based upon household size (calculated during eligibility). Be experiencing literal homelessness or facing imminent housing loss.
An Integrated Approach to Healthcare
In addition to treating individuals with serious mental illness, CBI also treats those who are pursuing services for their general mental health. These individuals are traditionally treated at one of our PCMH clinics. Our integrated treatment models allow us to treat a population that relies on other resources to get treatment.
