CBI is passionate about supporting the men and women in our community who struggle with their general mental health or those have been diagnosed with a serious mental illness (SMI). We know that with the proper combination of care and services, they can—and do—thrive.
We offer a statewide infrastructure that leads to quick intervention for people who are in great need. We do this with big hearts and no time limits, because the road to mental wellness is not a straight path.

We Often find that our patients come to us with mental health concerns that co-occur with other symptoms. This means they have multiple diagnoses: mental health disorder alongside substance misuse, perhaps homelessness and other physical ailments.
With co-occurring symptoms, it is imperative to treat the whole person, from head to toe, with our integrated approach to healthcare, so that patients might thrive. Just like the pieces of a puzzle, unless each one finds its home, the puzzle is left unfinished. This is also true when working to bring people to mental health and physical wellness.

Referral Process
Who is eligible?
We accept all patients. We are contracted with all Arizona Complete Care health plans and many private insurances. If a patient is uninsured or underinsured, we help them gain access and connect them with relevant resources without compromising care.
How long will the referrals last?
In most cases, referrals will be processed within 2-3 days and an initial appointment made within 7 days of the receipt of the referral.
What is the referral process?
A referral with CBI can be initiated by completing a secure referral form over phone or fax. We request the following information to be able to coordinate the referral efficiently:
- Member demographics
- Release of Information (ROI)
- Reason and scope of referral requested
- Referral type and requested timeframe for follow-up
- Payer information
- Reason and scope of referral requested
- Patient health and medication history
