PROJECT FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS
Meeting Individuals Where They Are At
CBI's PATH program, or Projects for Assistance in Transitioning from Homelessness, provides services to people with serious mental illness, including those with co-occurring substance use disorders, who are experiencing homelessness or at imminent risk of becoming homeless.
This federal grant program provides outreach and case management services, working to engage individuals in the community and help them connect to the Arizona Behavioral Health System.

Alcohol or drug treatment
Assistance with identifying and securing appropriate housing
Case management
Emergency assistance
Outreach
Habilitation and rehabilitation services
Referrals for primary healthcare, job training, education, and housing
Screening and diagnostic treatment services
Staff training
Supportive and supervisory services in residential setting
Accessing Care & Services
Who is eligible?
Adult Title 19 and Non-Title 19 members determined to have a Serious Mental Illness enrolled with Mercy Maricopa.
