Forensic Assertive Community Treatment, or FACT, utilizes the Assertive Community Treatment evidence-based practice of delivering a full range of services to people who have been diagnosed with a serious mental illness and are a medium to high risk to recidivate back into the criminal system. FACT’s goal is to work closely with patients to plan for re-entry back into the community and reduce interactions with the criminal justice system by providing the patient with intensive treatment services to help them live successfully in the community.
The CBI FACT Teams have mutual expectations between the team and its patients that are met collaboratively. These expectations include face-to-face engagements at least 4 times per week, creating and developing support systems and maintaining home visits, all to help identify and work toward patient goals.

Honoring our recovery-based origins, CBI’s Forensic Assertive Community Treatment (FACT) Teams include staff who have backgrounds as Peer Support Specialists, each possessing unique experiences with the population being served.
Accessing Care & Services
Who is eligible?
Individuals diagnosed with a serious mental illness who meet the Assertive Community Treatment screening criteria AND score medium/high on the Risk to Recidivate screening facilitated by the criminal justice system.
Appointment Scheduling
Once a referral is received, the team will contact the member and schedule an appointment for a screening. The team will make 3 attempts to complete the screening before closing out the referral. All members need to agree to the FACT level of care.
What is the appointment process?
All referrals for FACT teams originate from the criminal justice system (probation, parole, judges, etc) and are sent to Mercy Care Plan (MCP) for review. Once MCP screens the referral, the referral is sent to the FACT team. The FACT team will then schedule a screening with the member to ensure the member meets criteria and agrees to the services.
