Assertive Community Treatment (ACT), is an evidence-based practice that delivers a full range of services to people who have been diagnosed with a serious mental illness (SMI). ACT’s goal is to give patients adequate community care and to empower patients to live independently in the community.
CBI ACT Teams have mutual expectations between the team and its patients that are met collaboratively. These expectations include face-to-face engagements at least 4 times per week, creating and developing support systems and maintaining home visits, all to help identify and work toward patient goals.

Honoring our recovery-based origins, CBI ACT teams include staff who have backgrounds as Peer Support Specialists each possessing unique experiences with the population being served.
Accessing Care & Services
Who is eligible?
Individuals diagnosed with a serious mental illness that meet the Assertive Community Treatment screening criteria.
Appointment Scheduling
Once a referral is received, the team will contact the member and schedule an appointment for a screening. The team will make 3 attempts to complete the screening before closing out the referral. All members need to agree to the ACT level of care.
What is the appointment process?
A full referral packet needs to be sent to [email protected] from the current clinical team. The referral will be reviewed and the team will complete a screening to determine if the member meets the criteria for ACT services. If the member meets criteria, a full intake assessment will be scheduled.
