Community Bridges, Inc. will open the West Valley Access Point and Transition Point on September 7, 2011 at 3 pm. Please feel free to circulate the attached flyer that provides basic referral information and an overview of services that will be offered in both facilities. Access Point
Intake & Assessment – A front door to the behavioral health system offering assessment for ongoing services, brief intervention, and coordination of care through contracted network providers.
Transition Point
Level III time-limited residential (3-5 days) – 16 beds for individuals (18 and older) who need targeted overnight support or planning for transition back to the community.
