Outpatient Services
Community Bridges’ Outpatient programs provide numerous co-occurring capable and enhanced services to accommodate individuals and help families acquire the skills they need to reclaim their lives from the grips of addiction. Our Outpatient programs, located throughout Maricopa and Gila counties, are primarily behavioral and cognitive in nature, and are modeled after nationally recognized “best practice” modalities. All of the Outpatient programs provided by Community Bridges have the family member in mind and make every reasonable effort to actively engage them throughout the treatment process. Another key factor of our Outpatient programs is the goal of coordinating care with either existing professional services involved in the individual’s life or identifying those resources necessary to assist in creating successful outcomes. It is important to note that each of our programs have been designed and are maintained to provide individualized treatment based upon the individual’s presenting level of need. The following are some of the programs currently offered by Community Bridges Outpatient Services Department
Center for Excellence:
The Center of Excellence is a collaborative interdisciplinary coordinated effort between Community Bridges, the Maricopa County Drug Court and the Regional Behavioral Health Authority (RBHA) of Maricopa County to provide Methamphetamine Specific Treatment utilizing a unique therapeutic treatment model developed by the Los Angeles based Matrix Institute. Participation in the program can last up to 12 months based upon the intensity of services required to meet the individual’s needs in conjunction with determinations made by the treatment team. The Center for Excellence also utilizes the “best practice” approach ofContingency Management (positive reward for positive actions) to provide positive rewards for group attendance and for negative results in urinalysis drug screens.
Community-Based Treatment Services:
Community Based Treatment Services brings co-occurring capable services into areas of the community that generally have the least amount of services but have the greatest amount of need. Community Bridges unique partnership with Central Arizona Shelter Services (CASS) allows us to co-locate full time licensed counselors, Peer Support Specialists, Behavioral Health Technicians, and Transition Coordinators at the Homeless Campus Lodestar Day Resource Center (LDRC), Steele Commons Permanent Housing Program, and CASS itself. This ability to co-locate our team of addiction professional enables Community Bridges to work with other homeless services providers and housing providers to create a “one stop shop” for all of the individuals’ behavioral health needs.
Intensive & Standard Outpatient Services:
Our Intensive and Standard Outpatient programs provide co-occurring capable treatment and offer a minimum of 6.0 to 9.0 hours of group per week for a minimum of 12 weeks of treatment. These programs are usually utilized as a step down from a higher level of care or utilized for the individual who has not accessed residential treatment services and requires a higher intensity of services. A recommendation as to the appropriate intensity of service will be made by the attending clinician at the time of admission to the outpatient program. Crisis stabilization, inpatient medical detoxification, individual counseling, family, and peer support services are all available to those in this program.
Criminal Justice Outpatient Services:
Community Bridges is a contracted to provide outpatient treatment, assessment, and evaluation services to individuals referred to our agency by the Criminal Justice System. Specifically we provide direct service to those individuals referred to us by the Maricopa County Drug Court, Maricopa County Pre Trial Services Agency, Corrections Officer/Offender Liaison (COOL) Program, and individuals who are on probation and authorized to receive services through the Drug Education and Treatment Fund (DTEF). The dose and duration of treatment prescribed to each patient is based upon specific program criteria established by the presiding court, assessing entity of the court, or in coordination between our clinical team and criminal justice professionals. Ongoing care, reporting of progress and coordination of care is facilitated by the attending clinician in collaboration with the assigned probation or parole officer. As with our other Outpatient programs the modalities utilized are “best practice” models and are all co-occurring capable with the ability to provide crisis stabilization, inpatient medical detoxification, individual counseling, family, and peer support services are all available to those in these programs.
The Maricopa Drug Court Program is unique in the fact that like our Center for Excellence Model it offers 52-weeks of ongoing treatment and supports. Although the core of primary treatment may be completed within 32-weeks the individual will continue to attend Relapse Prevention and Aftercare until he or she has graduated from the Drug Court itself. In addition to this extended level of support, Community Bridges has designated employees who work as Court Liaisons. Our Court Liaisons are assigned to each of our Drug Court patients’ and attend weekly court sessions, staffings, and submits monthly reports to both the probation department as well as the presiding judge. This additional support helps ensure that there is a clear line of communication and coordination of care between criminal justice and Community Bridges treatment professionals.
